Home Health Aide
A certified home health aide can perform many tasks during the day or evening as needed, including assistance with bathing, dressing and/or grooming, meal preparation, medication reminders, companionship, light housekeeping, laundry, accompanying to errands or appointments, escort to dining room and/or resident activities, and pet care.
All engagements with a Registered Nurse (RN) begin with an initial assessment, consisting of a physical assessment with medical history and medication review, development of an individualized service plan, and review of disease-specific education materials. Following the initial assessment, clients will receive quarterly visits – during which time the RN will review medication, provide a physical assessment addressing any changes or issues, conference with the Home Health Aide to discuss possible issues, and provide a service plan for the next quarter.
Additional nursing visits are available for a variety services, including wellness visits, medication management and pre-pour, administration of injectable medicines, education and monitoring of a new or unstable diagnosis, diet review and management related to specific diagnoses. RNs are also able to follow up with family members following visits if necessary.
A homemaker can provide assistance with light housekeeping, laundry, and meal preparation.
Companionship services from Preferred Gold provide you with a helping hand and pleasant presence to help make your daily life a little less isolated. These services are not meant to replace the presence of a nurse, and our companions are not registered nurses, but they are able to do anything that a friend might lend a hand with.
An escort provides assistance to the client when they are discharged from a facility and accompanies them to their home. An escort can also provide support when a client is going to an appointment.
Personal Care Services
A Personal Care Assistant will assist with personal care needs including dressing, toileting and ambulation. The Personal Care Assistant can also provide light housekeeping.
Our Case Management professionals work closely with our nurses to facilitate the recommended care plans to assure the appropriate medical care is provided. This includes planning, monitoring, and coordinating health care services given by us and/or other health care providers to achieve the goal of recovery and relapse prevention.
Lifestyle Support Specialist
Our staff ’s focus is to be sure we provide the best client experience possible, enabling clients to continue to pursue their passions, professional business, and social relationships while facing a healthcare/aging situation. Additional focus entails coaching caregivers on details related to activities of daily living, whether or not related to a health challenge.
Our Nutritional Counseling program allows clients to meet with a Registered Dietitian to assess his or her usual dietary intake and identify areas where change is needed. Clients will be given a plan of action to assist in their treatment to ensure the highest level of quality care.
Our Travel companions assist in escorting clients when they need to go outside of the home including, but not limited to, medical appointments, functions, shopping, etc. This provides clients with the comfort of knowing they have an extra hand at all times.
Our team of rehabilitation specialists is dedicated to assisting clients on their journey to wellness. They are committed to assisting clients in regaining their health and independence through an array of rehabilitative therapies. Our rehabilitation services include Occupational Therapy, Physical Therapy, and Speech Language Pathology.
